Mission and History

Sun Life Family Health Center is a not-for-profit organization providing health care to Casa Grande, Eloy, Coolidge, Maricopa, Oracle and San Manuel. Our mission is to deliver comprehensive, high quality services to all people in need of affordable, accessible and culturally effective primary care.

Sun Life Family Health Center is Pinal county’s largest provider of primary health care services. Governed by a Board of Directors representing the patients and communities served by the organization, Sun Life serves over 32,000 patients, 30% of whom are children. What makes us different is our unfailing concern for the well-being of our patients, and our willingness to provide the best possible experience for every person that walks through our doors.

Sun Life Family Health Center is accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), considered to be the “gold standard” for quality in health care. JCAHO accreditation is voluntary, and means undergoing a challenging, and comprehensive evaluation.

Sun Life has made significant efforts to review and improve the key factors that can affect the quality and safety of patient care. This standard is met and maintained by only 5% of the community health centers nationally, and Sun Life is one of only two community health centers in the state to achieve and maintain this accreditation.

Dental Services

Dental care for you and your entire family and professional cleaning by our registered hygienist. Dental services include routine exams, fillings (including white fillings), teeth whitening, crowns, bridges, extractions, root canals, full and partial dentures, and orthodontics.

North Country Community Health Center

Vision & Organizational Goals

North Country’s vision is to be the primary care provider of choice throughout northern Arizona. The North Country Board of Directors and Management Team recently participated in a strategic planning process that resulted in six overarching strategic goals for the organization. Specific objectives and activities for each of these strategic goals are becoming part of a new and dynamic Five Year Strategic Plan for FY2005-2010. The theme of this new Five Year Strategic Plan is “Moving from breadth to depth; reaching new heights in quality of care through data driven, evidence based process improvements which include evaluation and monitoring.” The six overarching goals are as follows:

Dental Services

At North Country HealthCare we understand it has been a long time since some people have been to the dentist; some people have never been. We will work with you to understand what’s happening in your mouth so we can eliminate disease, restore good function and improve smiles.

North Country HealthCare’s Dental team is always accepting new patients! Your dental health affects your overall health, and North Country’s goal is to provide comprehensive dental care at an affordable price.

The North Country Dental Clinic has four licensed, board certified, general dentists and two registered dental hygienists on staff with over 100 years experience.

North Country’s Dental Clinic provides preventive care such as teeth cleaning, comprehensive exams, x-rays, fluoride treatments, and oral cancer screenings. North Country also provides restorative care such as fillings, crowns, bridges, and dentures. Additionally, North Country’s Dental Clinic can accommodate patients with special needs such as physical or developmental disabilities.

Sage Memorial Hospital

Navajo Health Foundation /Sage Memorial Hospital is the nation’s first Native – managed comprehensive health care system; it started in July 1974 when health care management was turned over to the Foundation from the local Presbyterian Church Mission after seven decades. Since then, a predominantly Navajo Board of Directors and staff have worked with great dedication to overcome health problems unrivalled anywhere in the nation.

Dental Services

Sage Dental Clinic Provides services to persons of all ages. With our full-time dentists and hygienist we are committed to providing you with professional and quality services.

Sage Dental services a wide area:1,800 square miles and 22,000 patients. Over 10% of the Navajo Nation.

Sage Dental offers services at field clinics in Greasewood (two chairs) and Sanders (four chairs) with a rotating dentist from the main clinic 4 times a week. Our main clinic in Ganado consists of eight chairs and five dentists. We are reaching only 20% of our potential need. The continued growth of the Dental Clinic has been a major project for Director Andemichael. Constant search and recruitment of dentists with talent and a willingness to practice in a rural and remote environment will remain a priority.

With the rising demand, Sage Dental only accepts new patients one day a month. Walk-ins are accepted every day for emergency visits.

Sage Dental has sealant programs with the local schools. We offer diagnosis and treatment for children so that good dental habits and cavity prevention can begin at an early age.

In 2002, Sage Dental had a total of 8,426 clinical visits. That number continues to climb. We are focused on becoming the preferred dental provider in our area. Our patients receive comprehensive treatment (exams, x-rays, cleaning, cavity fillings, other).

Our staff consists of eleven assistants, three receptionists and a Secretary/Accounts Representative. Sage Dental provides a comfortable office environment. Our staff is easy going, helpful and kind. Our dentists are down to earth and make the patients feel comfortable. We have a professional, dedicated and fun working staff.

Arizona School of Dentistry and Oral Health

Clinic

Unlike other dental clinics or dental franchises, our clinic is part of a dental teaching institution, the Arizona School of Dentistry & Oral Health. We offer a range of dental services like many private dental offices; in fact, our faculty’s expertise and available technology actually allows us to offer more services than many clinics.

We offer child and infant dental care, dental care for special needs patients, (i.e. patients with disabilities), and advanced surgical and non-surgical treatment options many clinics aren’t equipped or qualified to offer. The Dental Clinic has cared for more than 6,000 patients and our dental emergency service regularly sees between 10 and 15 patients per day. We currently provide compassionate care to approximately 300 patients every week.

Faculty

The Arizona School of Dentistry & Oral Health, part of A.T. Still University of Health Sciences, educates students to become technically adept, caring and compassionate dentists. Our dental teams are composed of dental faculty members and student doctors. Our faculty are experts, so you are assured you’ll receive high quality care using the newest technology and people-friendly, compassionate techniques.

Student Dentists

As a teaching clinic, student dentists are the heart of our clinic at the Arizona School of Dentistry & Oral Health, Arizona’s first and only dental school. Our student dentists work with supervising faculty to learn all the skills necessary before they treat patients. Students study for 3 years before they ever work on a human patient. Student training involves hundreds of hours of patient observation and practice on human simulators – complete with realistic teeth, gums, tongue, and jaws.

Students train for four years to become dentists. The first two years, students focus on health sciences, clerkships in general dentistry, and dental simulation exercises. In the third and fourth years, they participate in the on-campus clinic and practice in community-based health centers. Our dental degree program strongly emphasizes patient care experiences through simulation, integration of biomedical and clinical sciences, and problem-solving scenarios to achieve clinical experience and includes a strong component of public health, leadership, and practice.

The student dentist who will care for you has been carefully trained and screened. Your student dentist has been carefully supervised and has practiced her/his dental skills and techniques literally hundreds of hours in simulation laboratories.

Technology

The Dental Clinic is a state-of-the-art facility, and we use the latest technology to provide a range of services unavailable at most private clinics. We offer painless periodontal therapy, and our laser therapy uses light energy to modify tooth, bone, and gum tissue. Our students are among the first in the country to graduate with a certification in laser therapy. We also use an advance Cone Beam CT for imaging used in diagnosis, and our special needs unit uses customized equipment to provide care to special needs patients.

Maricopa Integrated Health System (MIHS) includes Maricopa Medical Center, the Arizona Burn Center, the Comprehensive Healthcare Center, the McDowell Healthcare Clinic, 10 community-oriented family health centers, and an attendant care program

Maricopa Medical Center is a premier training center for the nation’s physicians. Our medicine, surgery, pediatrics and ob/gyn programs, in particular, contribute to the body of knowledge of patient care. Maricopa Medical Center ensures that health care is available to all area residents giving paying and non-paying patients access to top quality medical care.

MIHS is the health care safety net for citizens of Maricopa County. The health system serves people of many races and nationalities who come from diverse cultures and speak several different languages. Many of the patients face major challenges, such as lack of health insurance, complex medical problems, and difficult socioeconomic situations. Caring for these patients demands special knowledge and sensitivity. MIHS is committed to giving culturally appropriate, sensitive medical care and helping our patients live healthier lives. MIHS provides care for all that enter its doors. Annually, MIHS has nearly 20,000 inpatient admissions and 300,000 outpatient visits.

Maricopa Medical Center is a full-service, 449-bed hospital that sees nearly 20,000 inpatient admissions every year. The hospital features advanced medical services including burn care, pediatric and adult emergency care, and has been recognized by U.S. News and World Report in its report on “America’s Best Hospitals” for excellence in respiratory disorders, urology, medical care, education and research.

El Rio Community Health Center

Since 1970, the El Rio Community Health Center has been working to provide quality, accessible health care. Today, more than 75,000 people in the Tucson community rely on the non-profit health center as their primary source of medical and dental care.

The El Rio staffs serve over 1,000 people per day, 70% of those served live at or below the federal poverty level. Dedicated to serving all populations including the underrepresented and uninsured, El Rio has become one of the largest non-profit, community health centers in the United States.

In the late 1960’s, neighborhood activists and the University of Arizona worked together to bring accessible and affordable health care to Tucsonans, who were being overlooked by traditional health care systems. Funded by a national “War on Poverty” initiative, a new concept was born, “Neighborhood Health Centers.”

Dental Services

El Rio offers dental services to meet the diverse needs of you and your entire family. Services include the full range of general dental care including preventive, restorative, simple oral surgery, crowns, bridges and complex periodontal (gum disease) care. In addition, our dentists provide children’s dentistry, oral surgery service (extractions, wisdom teeth), orthodontics (braces), and endodontic services (root canals).

An Urgent Dental Care service is available as well. These services are available to patients of all ages with insurance or on a sliding fee scale. We offer a reduced payment program for self-pay patients based on household income. For your convenience El Rio has three clinics to serve your dental needs.

The Clinic

Our Pascua Yaqui clinic is housed at the Hiak Hitevi Kari (Yaqui Healer’s House) at the tribal head quarters located at 7490 S. Camino de Oeste. The clinic has Family Practice and Pediatric services available along with special diabetic care through our Clinical Pharmacist program.

Profile

The Indian Health Service (IHS), an agency within the Department of Health and Human Services, is responsible for providing federal health services to American Indians and Alaska Natives. The provision of health services to members of federally-recognized tribes grew out of the special government-to-government relationship between the federal government and Indian tribes. This relationship, established in 1787, is based on Article I, Section 8 of the Constitution, and has been given form and substance by numerous treaties, laws, Supreme Court decisions, and Executive Orders. The IHS is the principal federal health care provider and health advocate for Indian people, and its goal is to raise their health status to the highest possible level. The IHS provides a comprehensive health service delivery system for approximately 1.9 million American Indians and Alaska Natives who belong to 564 federally recognized tribes in 35 states.

Winslow Indian Health Service

Winslow Indian Health Care Center is an ambulatory health center that serves an estimated 17,000 Native Americans – mostly Navajo.

Dental Services

Winslow Service Unit

The Winslow Service Unit, of the Navajo Area, of the Indian Health Service encompasses 1,600 square miles and serves over 14,000 people. The principal clinical site is the Winslow Health Center, a comprehensive ambulatory center accredited by AAAHC. Two additional clinics in Dilkon and Leupp are within 35 miles of the center supplement its services. In 2005 Winslow added a two chair mobile unit to assist its school-based programs. Winslow Dental Department employs seven dentists, three dental hygienists, twelve dental assistants, and five office staff. The treatment philosophy encourages preventive dentistry with an emphasis on patient responsibility. The service unit provides a full range of dental care to patients and annually sponsors dental sealant programs in elementary schools in the area.
